We welcome clients to self-refer without the need for a physician or third-party referral. Whether you are seeking therapy, assessment services, executive functioning coaching, or psychiatric consultation, you can begin your care with us directly.

A formal referral is required for psychiatric consultation with our psychiatrist. This referral must be completed by a family physician. Click on the form below to view and download.
